Sign ups for Douglas County vaccinations will be made available on the Douglas County Health Department's website every Friday at 4:30pm, unless otherwise stated by the Department.

You MUST live or work in Douglas County to be eligible to be vaccinated in this county.

The Douglas County Health Department as that patrons complete the forms below prior to their upcoming appointment at one of the vaccine clinics. Completing the forms listed below in advance of your appointment will expedite your check-in process at the clinic. The following documents can also be found at the library.
